File segment inexplicably overwritten -Sound Forge - PC

Our talking book studios use SF 6.0 Studio for mono voice recording of talking books. Incidents similar to the following have happened with both of our computers used for this work and have happened to different operators. "A passage, about 3 minutes long, was in its correct spot about 70 minutes into Side_2_Big_Red_Report.wav. The same passage was also about 6 minutes into Side_2_Big_Red_Report.wav and had overwritten the original text recorded there." This doesn't appear to be a paste from the clipboard because the passage begins and ends in mid-word--an edit we wouldn't use here. Any assistance is greatly appreciated. System a Pentium 4 2.8GHz, 1MB/800, 1GB DDR2 SDRAM, 533MHz. Drive is a 120GB IDE.
